- A white nowy-headed rectangular tablet set against a conforming black ground. A large weeping branch is carved above the inscription which is incised in roman-style caps of varying sizes with some sloped lettering. A carved shield of arms is placed just below the inscription tablet and overlaps the black ground. Some of the painted tincture is now lost.

The sculptor’s name is incised in caps on the front E side at the bottom of the ground:
